Then you will fill out a health history form which includes an updated list of your medications plus a consent form.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">The sessions are on Monday, Wednesday and Friday between 8 and 10 a.m. on the 2nd floor, at the east side of the building; enter through the heavy doors beside the physiotherapy clinic.
